Foyer Père Fiset, a 71-bed long-term care home located in beautiful Chéticamp, NS, is seeking a skilled Clinical Care Coordinator to provide hands-on clinical leadership and daily floor support for our nursing teams.
This role is essential to supporting safe, consistent, resident-centred care delivery through direct staff support, coaching, and real-time problem solving. The Clinical Care Coordinator works closely with the Director, Clinical Services and Director, Resident Care to ensure adherence to care standards, support staff development, and identify clinical risks requiring escalation.

Key Responsibilities
Clinical & Floor Support
-
Provide real-time floor support to CCAs and LPNs during assigned shifts
-
Act as the first point of escalation for immediate resident care concerns and staff support issues

Support adherence to resident care plans, clinical routines, and standards of practice

Assist with coverage coordination and immediate issue resolution on shift
Staff Support, Education & Orientation
- Conduct and support staff education including but not limited to - orientation, skill refreshers, and on-the-floor coaching

Provide guidance, feedback, and informal coaching to promote safe and consistent practice

Support the implementation of education sessions, clinical updates, and practice changes
- Create and/or find resources for ongoing education with Director, Clinical Services
Quality, Compliance & Safety

Monitor compliance with documentation standards, infection prevention and control practices, and safety protocols (IPAC duties and responsibilities/oversight)

Participate in quality improvement activities including audits, reporting, and follow-up on corrective actions

Support inventory processes and clinical supply management

Identify trends, risks, or recurring issues and escalate as appropriate
Collaboration & Communication

Maintain open communication between frontline staff and leadership

Escalate chronic, high-risk, or performance-related issues to the Director, Clinical Services and Director, Resident Care

Participate in interdisciplinary meetings and contribute to care planning and quality initiatives
Additional Duties

Assist in implementing clinical initiatives, policy updates, and education programs

Additional duties may be assigned as operationally required
Duties may be added, modified, or changed depending on operational needs.
Qualifications
-
Regulated health care professional (RN) in good standing with the applicable provincial regulatory body

Demonstrated experience in long-term care or a related nursing environment

Strong understanding of resident-centred care, clinical routines, and standards of practice

Excellent communication, organization, and interpersonal skills
-
Ability to coach, support, and guide staff in a respectful and accountable manner
-
Strong observational skills with the ability to identify risks and escalate concerns appropriately
- Wound care education or experience is an asset
